What you want changed about your lips decides which treatment fits, and which one does not. Volume, border definition, lip lines and a lip that thins when you smile are four different goals with different treatments.

What Changes About Lips Over Time

With age the lips lose volume, and the border that outlines them softens. The upper lip lengthens and rolls inward, so less of it shows at rest and when you smile. Fine lines settle into the skin around the mouth, a separate change from the lip itself.

These changes arrive in different combinations, which is why "fuller lips" is not yet a plan. Think of the goal as the address and the treatment as the route; the route cannot be chosen first.

Which Treatment Fits Which Goal?

Match your goal to one option and the limit it carries.

  • Volume: Restylane® hyaluronic acid filler adds fullness where it is placed. It does not change how the lip moves.
  • Border and shape definition: Filler placed along the lip edge sharpens the outline. It does not treat lines in the skin beyond it.
  • Lines in the skin around the mouth: Botox® or Dysport® relaxes the muscle that creases the skin so the lines soften. It adds no volume to the lip.
  • More upper lip when you smile: A lip flip with Botox® or Dysport® lets more of the upper lip roll outward. It does not make the lip larger.

Filler for Volume and Definition

Restylane® is a hyaluronic acid gel that adds volume where it is placed and binds water as part of holding it. Lip filler is not one material: approved sites, ingredients and duration differ by product, and your plan names the exact one.

Some volume change is visible right away, and swelling alters shape and symmetry for the first days, so the settled result is what counts. After a first filler treatment, a visit at 2 weeks checks healing and your early response.

Is a Lip Flip Right for You?

A lip flip uses a small amount of Botox® or Dysport® to relax the muscle around the upper lip. More of the lip then rolls outward when you smile. The lip flip and treatment of lip lines both fall outside these products' approved labeling, and we discuss that before treatment.

Weakening more muscle than intended can change how the lip moves, speaks or drinks from a straw for a few weeks. Careful dosing and placement reduce that possibility. The lip flip answers a different question from filler.

  • What changes: Filler adds volume and definition. A lip flip changes how much upper lip shows when you smile.
  • How the result develops: Filler shows change right away and settles as swelling fades. Botox® and Dysport® develop over several days, with full effect at about 2 weeks.
  • How long it lasts: Hyaluronic acid filler in the lips may last roughly 6 to 12 months, depending on the Restylane® product and your anatomy. It can fade sooner or last longer. A lip flip is temporary and shorter-lived than filler.
  • What each cannot do: Filler does not change how the lip moves. A lip flip adds no volume and does not change the lower lip.

Lip Lines Are a Different Problem

The fine vertical lines above and below the lips sit in the skin, and filler placed in the lip does not reach them. We assess those lines on their own, and the treatment for them is Botox® or Dysport®. Our Botox® and Dysport® page explains that visit.

Naturalness is the goal of conservative planning rather than a promise: your anatomy, the product, the amount and your healing decide the result. Asymmetry you started with may remain.

Devices and Materials We Do Not Use

Needle-free "hyaluron pen" devices are not approved by the FDA and have caused serious and sometimes permanent injuries. We never use them, and you should never attempt self-injection or use filler bought online. Permanent and non-hyaluronic-acid materials are a different conversation, because they are difficult to remove.

Before You Book

Approved lip filler indications begin at age 22. Active infection or inflammation at or near the injection site, including a cold sore, pimple, cyst, rash or hives, postpones treatment. Tell us about any history of cold sores, including tingling or an outbreak after a past procedure.

Tell us also about severe allergies, including any past life-threatening reaction (anaphylaxis) or multiple severe allergies. Tell us about allergy to numbing medicine (lidocaine) or other amide-type numbing medicines, bacterial proteins, or any filler ingredient. Also tell us about bleeding disorders and blood-thinning medication; a prescribed blood thinner is stopped only by the prescriber.

Tell us about treatment that weakens whole-body immunity and autoimmune or connective-tissue disease. So does a history of raised scars (hypertrophic scars or keloids) or a tendency to form them. Name any previous filler: the product, the site, the date, and whether permanent or non-hyaluronic-acid material remains.

Safety in pregnancy and breastfeeding has not been established, so treatment is deferred. For Botox® and Dysport®, a neuromuscular condition rules treatment out, because these products can cause serious weakness or swallowing or breathing problems. That includes myasthenia gravis and Lambert-Eaton syndrome.

ALS and other motor-nerve disorders are the same boundary. Dysport® may contain trace cow's milk protein and is not used where that allergy is known.

Afterward, expect swelling, bruising, redness, tenderness, firmness, itching, and temporary lumps or asymmetry, usually improving over days to about two weeks. Lip injection can also reactivate a cold sore. Some swelling, lumps, discoloration or delayed inflammation can last beyond that window, so tell us.

Less common are infection, persistent firm lumps (nodules), inflammatory lumps (granulomas), discoloration, movement of the product, and delayed inflammatory reactions weeks to months later. Rarely, filler enters or presses on a blood vessel and blocks blood flow, which can damage tissue or cause vision loss or stroke.

Seek emergency care immediately, and alert your injector, if any of these appear during or soon after treatment:

  • vision changes
  • stroke symptoms such as facial drooping or slurred speech
  • unusual, severe or escalating pain
  • skin turning white, gray, blue, dusky or blotchy

Contact our office promptly for any of these:

  • worsening redness
  • warmth
  • swelling
  • drainage
  • fever
  • a persistent lump

Bruising and color changes can be harder to see on darker skin, so report pain or color change out of proportion to the area. None of this is a diagnosis, and it does not replace an evaluation with your dermatologist.

Questions Patients Often Ask

Does Lip Filler Hurt?

Often, yes, briefly. Numbing cream goes on first and has time to work before your injections begin.

Is Time Off Work Necessary?

Not necessarily. Swelling and bruising are most visible in the first days, so schedule well ahead of any event. Our before and after article shows how settled results read.

Does Skin Tone Change the Plan?

Not necessarily. Both treatments are planned by your goal and your anatomy across skin tones. The one difference is how bruising shows, which the warning signs in Before You Book cover.

What If You Do Not Like the Result?

It varies. Hyaluronic acid filler can often be reduced with an enzyme injection (hyaluronidase), and complete reversal is not guaranteed.
