Insurance & Coverage

We participate with select plans from insurers such as Aetna, Blue Cross and Blue Shield (PPO and Options), Cigna, Corvel, Humana, Imagine Health, Multiplan, The Alliance, TriWest Healthcare Alliance, United Healthcare, and Zelis. Participation varies by plan and network.

We accept Medicare Part B and select Medicare Advantage plans.

We do not accept Medicaid or Medicaid Managed Care plans.

Coverage and benefits vary by plan and product, and some services—especially cosmetic/aesthetic—are self-pay only and not billed to insurance

We can give guidance on insurance eligibility as a courtesy, but it is the patient’s responsibility to confirm coverage with the insurer and to understand any copays, deductibles, coinsurance, and referral requirementsAny balance not paid by insurance—including non‑covered services—is the patient’s responsibility. Insurance participation can change; please verify directly with your insurer.

Evidence of current coverage must be presented upon scheduling an appointment or at check-in each time you visit our office. You may be asked to pay for your charges at the time of service if we cannot verify that your insurance is active, or if your insurance is not in-network. An itemized receipt will be provided for you to submit to your insurance company, for them to directly reimburse you. We cannot guarantee reimbursement.

It is your responsibility to understand your individual coverage or benefits including deductibles, coinsurance, and copays. For clarification on your policy, please contact your insurance carrier.
